rTMS is a non-invasive method of brain stimulation in which magnetism is used to alter brain (neuronal) activity. Depending in the type of illness, neuronal activity can be increased or decreased, by using specific parameters.
The rTMS machine consists of :
The coil is placed on the scalp at specific locations, determined using the 10/20 method of electrode placement. The patient is fully conscious and alert throughout the session. No form of anesthesia is administrated and procedure is totally non-invasive.
A point is marked on the scalp using the 10-20 marking system depending on the area to be stimulated. This varies from illness to illness.
The magnetic coil is placed on the head on the targeted area and stimulation is started after putting the patient in a comfortable position.
The treatment protocol consists of a series of magnetic stimulation for a duration of 30-45 minutes depending on the illness and protocol. The patient is expected to maintain the same position of the head for the entire duration of stimulation.
Usual number of sessions depends on the illness but can vary between 20-40 sessions.
No major side-effects are seen with rTMS. The only major side-effect possible is a seizure and the possibility is around 0.1%.
Response is usually seen within initial 10-15 sessions and distinct changes can be seen after the sessions are over. The effects of rTMS can last upto years without medicines. With medicines the results of rTMS have been superior.
